Takeover of BASE services in the event of death

First of all, we would like to express our sincere condolences. A loss is always painful. It goes without saying that we will do our utmost to keep the administrative formalities to a minimum. In this situation, all the formalities relating to the takeover can be dealt with online.
 
First of all :

By completing this form, you agree to a number of things. You declare that you, the transferee :

  • take over all BASE services from the chosen date, in accordance with the BASE general terms and conditions;
  • also take over any unpaid balance of the deceased, as well as any credit, modem guarantee or residual value of appliances purchased at the BASE Customer Price;
  • accept the transfer of personal data. This includes payment history, product data, usage data and previous account statements relating to the services transferred from the deceased;
  • accept the transfer of personal logins.

The IBAN number is indicated on your bank card and consists of the country code (BE, NL, FR) and a series of characters (numbers and/or letters).

Would you like to receive your invoices at the same e-mail address as above? Then you don't need to fill anything in. You will always receive your invoices by e-mail.

Takeover of mobile subscriptions

Do you also take over mobile subscriptions or prepaid cards? Enter the mobile numbers you are taking over here. For each number you can indicate which type of publication applies and whether you want to keep the number. If you do not wish to keep the number, we will automatically cancel it.

Do you include mobile phone numbers? You can choose to have your number(s) published in the White Pages or via 1307. If you make no choice, we will register your numbers as secret. Your details will not be published.

  • Secret number: not published in the White Pages and not available on 1307.
  • Unpublished number: not published in the White Pages, can only be obtained by calling 1307 with your address.
  • Unlisted number: not published in the White Pages, your number and address can only be obtained by calling 1307.
  • Normal number: published in the White Pages, and can be obtained by calling 1307.
